Skills Plus Registration

We require an application form with a parental signature along with a deposit of at least $40.00 and a post-dated cheque for the balance payable 21 days prior to your starting date.

How to Register

1. Get a registration form here.
2. Print the form.
3. Fill in the information.
4. Write your cheque(s) payable to Skills Plus Hockey Inc.
5. Mail in the form and cheques to :

6. Your cancelled cheque is confirmation of registration. Camp packages will be sent out three weeks prior to the start of camp. 

Note: There is a $40 admin. fee charged for cancellations.
